FAQs
We’re here to support clients, caregivers, and the community Whether you have been on this journey for a while or you’re just starting out, dealing with a brain injury can be an overwhelming experience. The Cognition Center is here to help with clear information for patients, family members and other caregivers or people who are interested in learning more about brain injury or brain health.
I’m a healthcare professional. How do I refer a patient?
Use our online contact form
Call us at (424) 259-1051.
When a patient joins the Cognition Center community, we invite all referring physicians or other healthcare professionals to become part of our treatment team by participating in ongoing meetings and evaluations as needed.
What is the typical care plan like?
Because we specialize in creative problem-solving, no two care plans are exactly alike.
Typically, the care plan uses diagnosis-specific clinical guidelines as a baseline, then it is personalized to reflect the participant’s life and goals. Our comprehensive approach also includes things like working with the participant and family on stress management, accompanying the patient on doctor’s appointments, and working with an individual’s employer on-site to facilitate a smoother return to work.
I’m a potential patient or family member. Do I need a referral?
No, you can contact the Cognition Center directly, and we’ll determine whether our program is a good fit.

Is home and community rehab the same as home health?
Not even close. The primary differences are:
Services provided – We deliver complex therapies with therapists who have extensive neurorehabilitation expertise.
Success criteria – We set different types of goals.
People served – Our average patient age is 43 years old.
In short: “home health” looks at medical outcomes and is primarily concerned with safety. We look at functional outcomes and our focus is on helping you do whatever matters most in your life.
What are the admission criteria for your program?
We generally work with individuals who are at least 18 years of age (with no upper age limit). We accept participants who are medically stable and show clearly definable rehabilitation. Symptoms related to the brain injury must have existed for at least 1 month. Participants need to have previously enjoyed riding a bike (of any type) and have a current desire to have some form of bike riding play a role in their life. Ideally, participants should come equipped with a solid support system (family or friends) to bolster their progress.
Are you a Medicare provider?
Unfortunately, Medicare does not cover home and community rehab programs.
Do you accept insurance?
At this time we are unable to accept insurance. If you would like to submit documentation to your provider to request reimbursement, we can provide that to you.
